Some documents and forms contained within this website have been created in the "portable document format" (PDF) using Adobe Acrobat software. This Acrobat software creates a replica of a document or form for printing or viewing on your screen. 

To use one of our Acrobat-created files, you need to download a free copy of the latest version of the Acrobat reader program. Once you download the software and install it on your computer, you can view the document or form. Moreover, if you choose to print the form, you will have an exact copy.  Please note that the Acrobat reader allows you to zoom in and out to allow you to focus on one part of a document.
